RISK MANAGEMENT PROGRAM FOR PARENTS AND GUARDIANS


WHAT IS RISK MANAGEMENT?

In Delta Sigma Theta Sorority, Inc. our Risk Management Program protects our organization, ensures the safety of all participants in our youth programs, evaluates our allocation of resources, and focuses on the enrichment of youth participants.

DELTA SIGMA THETA YOUTH INITIATIVE PROGRAMS

Delta Sigma Theta has three national youth enrichment programs designed to promote the development of young girls and boys. These include, but are not limited to, Delta GEMS, Delta Academy and EMBODI (collectively “youth initiatives”). The Montgomery County (MD) Alumnae Chapter (MCAC) also has reading tutoring, and a Science, Technology, Engineering, and Math (STEM) program.

Dr. Betty Shabazz Delta Academy
Youth program for young women ages 11-14 which focuses on social and emotional development, physiological transitioning, exposure to global ideas and learning opportunities.

Dr. Jeanne L. Noble G.E.M.S. (Growing & Empowering Myself Successfully) Institute
Youth program for young women ages 14-18 which focuses on promoting high levels of academic success, assisting young women in proper goal setting, developing compassionate, caring, community-minded women, emphasizing decision-making and planning for the high school years and beyond.

EMBODI (Empowering Males to Build Opportunities for Developing Independence)
Youth program for African American males ages 13-17 which is designed to address issues related to STEM education, culture, self-efficiency, leadership, physical and mental health, healthy lifestyle choices, character ethics, relationships, college readiness, fiscal management, civic engagement, and service learning.

​​Please review the Parent/Guardian Guidebook and complete the Parent/Guardian Forms Packet if you have a child or youth participant you plan to enroll in an MCAC Delta Youth Initiative Program.

How to Complete Application Forms and Submit to MCAC

Applications must be completed and signed in order to submit to MCAC. Incomplete forms will not be processed. Be sure to review all forms (including signature and initial fields prior to submission).

Step 1: Review the Parent/Guardian Guidebook.

Step 2: Download the Parent/Guardian Forms and complete. (See download instructions below)

Step 3: Access MCAC-DST Youth Initiative Parent/Guardian Submission Form.

Step 4: Upload completed and signed forms via submission form. *(Review all forms to ensure fields requiring initials or signature are complete)

Step 5: Click “Submit” button.

Instructions to download and complete application forms:

  1. Each application file is a fillable PDF. You will need PDF reader software installed on your computer to complete the form(s). A free PDF reader is available here.
  2. Select the relevant file and download it.
  3. Save file your computer. (Text added to PDF while opened in browser will not save)
  4. Open the file in its location on your computer.
  5. Complete and save the form.
  6. Click on the appropriate submission form link above to upload completed and signed forms.
